  • One time $99 initiation fee due at sign-up along with first month’s payment

  • Laser Memberships and the Combo Membership are 12 month contracts.  Penalties apply for premature cancellation.

  • The Botox and Xeomin Membership is a minimum 4 month commitment. Penalties apply for premature cancellation.

  • Start a new membership within 30 days of expiration and avoid paying the $99 initiation fee.
